MACLASS3.PDF - The Massachusetts Version of Page 2 for the Class 3 Medical Forms
This is a special Massachusetts Camp version of page 2 (for immunizations) that gets added to the standard Class 3 form - This File Size is 58,976 bytes. The Pioneer Valley Council has provided this and have used a state form, which they copied onto this part of the forms for all camp registrants. Out of state people who attend a Massachusetts camp need these immunizations too.
